We are seeking a Nurse Practitioner to join the HealthEast Oakdale Clinic in Oakdale, MN. This is a great opportunity to step into a small and well-run established practice with a collegial group of providers.
The clinic's primary care group consists of 7 Family Medicine physicians, 1 NP, podiatry and pharmacist. The clinic offers primary care services for all ages, from pediatrics to geriatrics, with a special emphasis on wellness and family care.
The CNP functions in the capacity of an Attending Provider in collaboration with a Physician. Responsible for primary health care of patients and promotion of wellness
Orders diagnostic tests such as imaging, pathology and lab tests
Ensures clinical excellence by ensuring quality, focusing on the patient experience, operational efficiency and clinical excellence
Works collaboratively with assigned physicians to manage those physicians’ panel of patients including acute visits and regular follow up of chronic disease management
Patient encounters average between 17 – 22 patients per day
100% outpatient, clinic only responsibilities
Clinic hours are Monday – Friday, 8:00 am-5 pm
A fully integrated electronic medical record (Epic) is utilized
