Practice Description

HealthEast is seeking a Nurse Practitioner to join our team in Maplewood, Minnesota to help manage a variety of chronic pain cases. Opening is part-time (0.6FTE), benefit eligible, with the opportunity to evolve into a full-time position. We offer a collegial team of 3 physicians and 4 nurse practitioners in a 100% outpatient setting. Within the scope of practice as defined by the regulatory bodies, the Certified Nurse Practitioner (CNP) functions in the capacity of An Attending Provider in collaboration with a Physician. Responsible for collaborative management of pain management/spine care conditions in patients with chronic acute care pain management. Primary responsibility for patient and family education related to diagnostic services, disease processes, treatments and a plan of care.


Detailed Responsibilities:

  • Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures and standards.
  • Actively participates in creating and implementing improvements.
  • Ensures assigned patients are properly and efficiently admitted and receive appropriate and responsive medical care according to their condition, symptoms and diagnosis. Develops a plan of care in conjunctions with other primary and specialty physicians and interdisciplinary staff to ensure quality care of patients. Complete effective and timely verbal communication with primary and specialist physicians.
  • Record and document health appraisal data necessary to maintain, coordinate and/or expedite required services.
  • Acts as a resource to health care staff and implements educational programs to improve management of patients/families.
  • Collaborates, consults and communicates with a multidisciplinary team of healthcare providers and staff throughout the continuum of care.
  • Helps to identify measurable, realistic and timely patient care outcomes, and participates in care management.
  • Communicates with patients and families to explain diagnosis, procedures, expected diagnostic or therapeutic treatments, prognosis and/or recovery periods, and potential or expected outcomes; and to obtain informed consent. Proactively communicates with other clinicians in order to affect a coordinated and timely transfer of information necessary to advance quality care and outcomes for outpatients.
  • Participates in assigned work teams, co-management committees and HealthEast department meetings to develop programs, protocols and assist in the overall improvement of quality of care and operations.
  • Documents patients history and physical (H&P) exams, diagnosis, treatment, orders and clinical progress in the chart, in order to allow for complete collection of comorbidity and complication information for use by hospital medical records and coding staff. Dictates discharge or transfer of care summaries to allow for timely communication. Returns pages and phone calls in a timely fashion to clarify orders, arrange transfers, follow-up on tests and answer patient, family and staff questions.




  • Must be AANP or ANCC Certified
  • Ability to obtain MN-APRN license and DEA
  • BLS and ACLS Certified
  • 3 to 5 years of NP experience preferred
  • Pevious experience with Pain Management/Spine Care or Neuro or Ortho highly preferred
